CHANGE, REPLACEMENT AND CANCELLATION OF REGISTRATION CERTIFICATE

The Data Controller is responsible for ensuring that the details on the registration certificate are accurate and correct. In the event of any amendments to the information, the Data Controller must submit an amendment application following the manual of Change of Particulars in Registration Certificate.

If the registration certificate is lost or damaged, the Data Controller may apply for a replacement registration certificate according to the following guidelines:

The application for the replacement certificate must be accompanied by a fee of RM30.00

If the Data Controller has ceased processing personal data, the Data Controller may submit a written request to the Personal Data Protection Commissioner to cancel the registration certificate, in accordance with the manual of Registration Certificate Cancellation.
